CONTRACT REQUIREMENTS

  • General Contract Requirements

    Loan repayment offered for Primary Care, Dental & Mental Health Professionals ONLY


    Must maintain 2+ years of service in HPSA area


    Must have direct patient care service


    • Part-time: 18 hours of direct patient facing time, 20 hours total
    • Full-time: 36 hours of direct patient facing time, 40 hours total

  • CME Requirements

    Awardees MUST take 2 courses (2 credits) per service year


    Service Year 1: 10/1/24-9/30/25 

    Service Year 2: 10/1/25-9/30/26

    • All CME courses from UNR and UMass must be selected from the "Coursework" page in "My Portal"
    • CME courses from Project ECHO may be accessed directly from their external website and do not need to appear on the "Coursework" page

  • Required Documentation

    CT SLRP awardees MUST SUBMIT THE REQUESTED FILES BELOW. If a member of our CT SLRP team is contacting you regarding outstanding documents; again PLEASE SUBMIT BELOW.

    • Proof of Licensure
    • Proof of Residency
    • Resume/CV
    • National Self Query Report
    • Loan Information Summary (MUST HAVE FULL NAME ATTACHED TO FILE SUBMITTED) - see attached image
