Instructions:
Once the course is completed and before you exit the course. Please make sure the CME/CEU certificate has your name, the name of the program, and the date of completion
Once you've completed your CME/CE - submit for credit approval below or send your certificate to ctslrp@uchc.edu

How to Receive CME Credit
Project ECHO programs require confirmation of attendance, so it is important to register in advance. ECHO programs will email an evaluation when the course is completed. Once the evaluation is submitted, participants are redirected to the Project ECHO Portal to input a code sent by ECHO to receive CME/CEU credit.
UMass will email a program evaluation upon completion of the course. Once the evaluation is submitted, participants will receive an email with the certificate of completion.
CT AHEC requires participants to take and pass (70% or higher) a questionnaire to show what you learned from the program. Once completed, the participant will receive an evaluation. The questionnaire and the evaluation must be completed to receive the CME/CE credit. The certificate will be sent via email within 7 days of completion

Connecticut Children's is offering coursework via "My Portal" - all users will need to create a free account from Eeds (linked to any Connecticut Children's course listed on "My Portal"). Once completing the course, awardees MUST obtain the certificate before submitting to the CME/CEU Upload page or by emailing our team at ctslrp@uchc.edu
